Please print and fill out the request form attached below and submit it along with your civil id by visiting the Branch or scan and send by email to Retailkuwait@dohabank.com.kw

The certificate will be issued within 5 business days from the date submitted subject to the Bank’s criteria for certificate issuance.

Kindly visit the branch in person with your Civil Id to collect the certificate.
